Release of Medical Information
All matters discussed and recorded during an office visit or examination are confidential. Dr. Hissong’s office will not release any information to anyone without your permission. Release of your records to insurance companies, your employer, or any representative, require your written permission. Also, release of information to spouse, parents (except in the case of minors), children, siblings, etc. require the same permission. If you wish us to discuss your medical condition with another/others, please indicate this on our permission form. If you invite another person into the visit with you, you are automatically granting permission for that person to be included in the discussion of your health.
